How To Refer

Eden Homes Notts provides temporary supported accommodation for single-room occupancy in Nottingham and accepts applicants aged 18 and over with housing-related support needs.

There are a number of ways to refer applicants to Eden Homes Notts:  

--> Referral from Health or Adult Social Care Services  

--> Client Self–Referral  

--> Referral by a Housing Officer  

--> GP Referral

--> Referral from Other Services and Organisations   


In order to make a referral, we would need to receive a completed Eden Homes Notts referral form. A referral form can be requested by emailing admin@edenhomesnotts.org

If referred by an organisation/service, the referring agent should complete the form with the agreement and involvement of the applicant.

Once the completed referral form is received, Eden Homes Notts will carry out an assessment and contact the referring agent/applicant to provide the outcome.

If the application is successful, a support worker will be allocated. A Support Plan will be set out with the agreement of the applicant.
